Criteria for Outstanding Achievement in a Clinical Practice

AWARD FOR OUTSTANDING ACHIEVEMENT IN CLINICAL PRACTICE

The purpose of this award is to give public recognition to APTA MA’s physical therapist and physical therapist assistant members who are outstanding, clinicians, dedicated to their patients, their professions, and to sharing their knowledge with others.  It is to be awarded whenever the APTA MA Board of Directors believes an appropriate individual should be so honored.

A. Qualifications for Physical Therapists

  1. Candidate shall be a member of APTA MA.
  2. Candidate has been engaged in extensive clinical practice for at least seven years.
  3. One whose professional allegiance has been first, and foremost to the patient.
  4. One who, on behalf of their profession, has participated actively in Chapter activities, giving generously and willingly of his/her time, and effort.
  5. Candidate may be a staff physical therapist or clinical educator.
  6. Candidate’s primary role is clinician or 50% of primary job responsibility is devoted to patient care.


B. Qualifications for Physical Therapist Assistants

  1. Candidate shall be a member of APTA MA.
  2. Candidate has been engaged in extensive clinical practice for at least seven years.
  3. One, whose professional allegiance, has been first and foremost to the patient.
  4. One who has worked effectively as a team, with physical therapists.
  5. One who, on behalf of their profession, has participated actively in Chapter activities, giving generously and willingly of his/her time, and effort.
  6. Candidate may be a staff or Senior Physical Therapist Assistant.

C. 3 letters of support should accompany the nomination.
